In addition, this includes patient information, which may be disclosed to my presence in a ballad health centre. I understand that this information is treated in the same way as all other patient information and that it offers the same level of confidentiality. In signing this agreement, I understand that respecting the confidentiality of patient information is a precondition for the continuation of my relationship with Ballad Health. I therefore agree to preserve the confidentiality of all patient information, including patient names, addresses, telephone numbers, social security numbers, diagnoses, pharmacological and surgical interventions, as well as the results of tests that I can know as part of my participation in each ballad health centre. I understand that any breach of confidentiality, including but not limited to inappropriate use or disclosure of patient information, is a violation of the Ballad Health Directive, and such an infringement will lead to appropriate action, including terminating my relationship with Ballad Health. In addition, such an offence may be considered a violation of federal provisions and is investigated by the Citizens` Rights Office. Our commitment to patient confidentiality is reinforced by data protection and security rules established under the Health Insurance Portability and Accountability Act („HIPAA“). These rules require that Ballad Health and those exposed to protected health information be required to keep patient information confidential. Ballad Health is committed to ensuring the confidentiality of patient information and to complying with all government and/or federal patient information protection rules.
Ballad Health has a long tradition of protecting patient information privacy, including patient names, addresses, phone numbers, social security numbers, diagnoses, pharmacological and surgical interventions, and tests and outcomes related to the provision of patient care to each ballad health facility.
